Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Uses and Disclosures

HelpStar Behavioral Health may use and disclose protected health information (PHI) for treatment, payment, and health care operations, and as otherwise permitted or required by law. TODO(client): detail per HHS model notice; add Part 2 (42 CFR) provisions if substance use disorder records apply.

Your Rights

You have rights regarding your PHI, including the right to access, request amendments, request restrictions, request confidential communications, receive an accounting of disclosures, and obtain a paper copy of this notice.

We are required by law to maintain the privacy of PHI, provide this notice, and follow its terms.
